yeah it does happen when you revive like Auto-Regen, Auto-Protect, Auto-Shell, Auto-Reflect

i had same problem,you have to board ship,then select to go to bikanel island(oasis) then leave 1 screen,then go back in to oasis,then cactaur will be seen to use save sphere and board ship,he will then be found on top deck,but if yuna is waiting on top deck then you have to move story on first,then do this.
